Your Training Records Are Lying to You

And the next recall, injury, or failed audit will prove it. Why the industrial world must pivot from documentation to verification.

Last month, we watched a trainee perform a flawless maintenance procedure on a surgical stapler. Textbook execution. Smooth hand movements. Confident pace. The supervisor signed off. Certified. Ready for the floor.

Except the trainee never sterilized the device.

Steps 15 and 16—submerging the anvil in sterile solution and wiping down surfaces—simply didn't happen. The video jumped straight from inspection to troubleshooting. In a training context, that's a failed assessment. In a surgical suite, that's a patient infection. In a courtroom, that's exhibit A.

The supervisor missed it. Of course they did. They'd watched 47 other trainees that week. They trusted the rhythm of the procedure. They saw what they expected to see.

This isn't a training problem. It's an evidence problem.

The Comfortable Lie of "Trained"

The gap between these requirements and standard practice is a liability sitting in plain sight. When an auditor asks "How do you know John can perform this sterile procedure correctly?" and your answer is a signature from eight months ago, you're one follow-up question away from a finding.

The regulatory posture is shifting from "show me your records" to "prove they can actually do it."

Force 3: The cost of getting it wrong has become intolerable.

A single assembly error in high-value manufacturing: $200 in scrap. Multiply by a 0.25% error rate across 10,000 units: $500,000 annually. One avoidable error. One line item that nobody tracks because training is "someone else's budget."

In field service, every callback is a margin killer—truck roll, labor, fuel, plus the opportunity cost of a job you didn't get to do. In medical device sales, one fumbled surgical demo doesn't just lose a contract. It plants doubt with every surgeon in that hospital network.

These costs are real. They're measurable. And they're directly traceable to the gap between "documented" and "verified."

What Changed: Machines Can Finally See

For decades, we've accepted human observation as the only option for evaluating physical skills. We digitized everything else—inventory, scheduling, quality metrics—but skill verification stayed analog. Someone watches, someone signs, someone files the paper.

That constraint no longer exists.

Multimodal AI can now watch video of a procedure and understand what it's seeing. Not just transcribe audio or detect objects—actually reason about whether the steps were performed correctly, in sequence, with appropriate technique.

The worker hits record to prove competence. The AI confirms it or identifies exactly where they fell short. The result is timestamped, stored, and audit-ready.

The Shift: From Knowledge Management to Skill Verification

The last decade produced excellent tools for storing information. SharePoint. Notion. Wikis filled with SOPs that nobody reads. Video libraries that employees scrub through once and forget.

We don't have a knowledge storage problem. We have a performance verification problem.

The question isn't "can your workers access the procedure?" It's "can your workers perform the procedure?" These are completely different questions, and we've been answering the wrong one.

This distinction matters because it reframes where you invest. Stop spending money on better content delivery. Start investing in closed-loop competency validation: capture the expert standard, let trainees practice against it, verify their execution through objective assessment, and use the data to identify where your procedures—not your people—are failing.

When 40% of trainees fail the same step, you don't have a training problem. You have a process design problem. Good verification surfaces that signal. Attendance records bury it.

What Objective Evidence Actually Looks Like

Picture an FDA auditor asking about a specific production batch:

"How do you verify that the operator on shift was qualified for this procedure?"

The old answer: "We have a signed training record from their initial certification."

The new answer: "We have a video-verified assessment from that week, AI-scored with 100% on all critical steps, timestamped and stored in our compliance system. Would you like to see the specific evaluation?"

One of these answers invites follow-up questions. The other closes the inquiry.

The companies that make this shift will stop treating training as a compliance checkbox. They'll start treating it as a quality control mechanism. And when their competitors face the next recall, the next injury investigation, the next failed audit, they'll have something their competitors don't.
